Proxy Access

In compliance with HIPAA laws and legal privacy, proxy access is available in three categories:

Minor Proxy access – Parents/guardians can set up a minor account allowing access to view and manage a child’s account until the child turns 12.

Young Adult Proxy access – Parents/guardians can have a limited FollowMyHealth account which allows secure messaging with the providers care team.

Adult Caregiver Proxy access – Medical Power of Attorney designees can request an account allowing access to view and manage a dependent adult accounts.

For Child Proxy Access

  1. Click here to print the child proxy form. Please complete this form then fax, mail or drop off the form along with a copy  of your photo ID at your primary care provider’s office.
  2. You will then receive an email invite. Click here for instructions on setting up an account with the email invite.

For Adult Proxy Access

  1. Click here to print the adult proxy form. Please complete this form then fax, mail, or drop off the form along with a copy of the legal documents stating medical power of attorney at your primary care provider’s office.
