Patient Portal Information

Our Patient Portal makes it easier for you to participate in managing your health care. You can use a computer or your mobile device to view, track and update your personal health record; view and update your personal information; review past appointments; request new appointments; request prescription renewals; view most of your test results and communicate securely with your provider.

To activate your account, just contact your provider’s office and request an email invitation from noreply@FollowMyHealth.com. (If you receive the invitation, please check your Junk Mail or Spam folder.) Once you have received your email invitation, simply click the link in the email message to begin the registration process.

For patients younger than 18 years of age: FollowMyHealth allows a parent/guardian to set-up a proxy account. Multiple children can be linked to one parent/guardian account. For all minor proxy accounts, a minor proxy release form must be completed, signed and returned to the office with a copy of the parent/guardian driver’s license. Once the form has been returned, a proxy invitation will be sent to your email address, with instructions on how to set up your account.
