Anne Chamberlin,
Doctor of Medicine

Dr. Anne Chamberlin is a dedicated family medicine physician at Western Colorado Physicians Group (WCPG) and Family Centered OB, divisions of Primary Care Partners. She believes in providing patient-centered care that supports your family through all stages of life. In addition to providing comprehensive adult primary care, Dr. Chamberlin has a passion for helping families grow, and her practice includes prenatal, obstetrical, postpartum, and pediatric care. She is committed to being your partner throughout the entire journey of life, offering consistent and personalized care.

Originally from New Mexico, Dr. Chamberlin completed her Family Practice Residency at St. Mary’s Medical Center in Grand Junction, solidifying her commitment to the Western Colorado community. She holds an undergraduate degree in Biology from Lewis and Clark College and a medical degree from Oregon Health and Science University, where she also gained extensive research experience and numerous publications.

Outside of her practice, Anne enjoys the active lifestyle that Western Colorado offers, spending quality time with her family biking, hiking, camping, rafting, and skiing.

Craig Hughes,
Doctor of Medicine

Dr. Craig Hughes has been with Western Colorado Physicians Group, a division of Primary Care Partners, since 2014. He graduated from the University of Utah with a Ph.D. in Physical Chemistry and the University of Utah School of Medicine with his medical degree.  Prior to joining Primary Care Partners, Dr. Hughes was in private practice in Salt Lake City, Utah. 

Dr. Hughes is committed to the health and wellness of his patients. He sees patients of all ages and his clinical interests include whole-person care and prevention through diet and exercise. He approaches healthcare as a partnership with his patients and focuses on a 360-degree view of one’s health to provide optimal physician care. 

Dr. Hughes loves to downhill ski and you can find him enjoying Colorado’s slopes during the winter months when he’s not working.

Joshua Hulst,
Doctor of Medicine

Dr. Joshua Hulst has been with Western Colorado Physicians Group (WCPG), a division of Primary Care Partners, since 2016. He received his bachelor’s degree from Bethel University, his medical degree from the University of Iowa Carver College of Medicine, and trained in Family Practice at St. Mary’s Medical Center. 

Dr. Hulst has practiced in a number of underserved foreign and domestic locations. He was originally led to medicine while working in a medical hospital in Ghana, West Africa.   Prior to his career as a physician, he worked for the United States Forest Service as a wild-land firefighter. 

Dr. Hulst works to bring health to his patients through a personalized approach to the care of the whole person. He has particular interests in advanced migraine headache treatment, dermatologic procedures, and vasectomies.

When he is not at the clinic, Dr. Hulst can be found chasing his family and dog around their yard, and hiking and camping.

Brett Jackson,
Doctor of Medicine

Dr. Brett Jackson is a Colorado native from the San Luis Valley in Southern Colorado. He considers La Jara his hometown, though he was raised just outside of town on an alfalfa circle in the close-knit rural community of Bountiful.

Dr. Jackson earned his medical degree from the University of Virginia in 2022. His passion for medicine began early, influenced by his parents—his father, a physician assistant in family medicine, and his mother, a nurse. Growing up in a “medical family,” he witnessed firsthand the importance of compassionate care and the value of strong patient-provider relationships. Inspired by his father’s dedication and long-term connections with patients, Dr. Jackson was drawn to a career in primary care.

Known for his laid-back and approachable demeanor, Dr. Jackson is committed to creating a welcoming, supportive environment for every patient. He is bilingual and provides care to both English and Spanish-speaking patients.

Outside the office, Dr. Jackson enjoys spending quality time with his family. Together, they love hiking, exploring local parks, trying new restaurants, attending plays and sporting events, and traveling.

Lisa Roberts,
Doctor of Medicine

Dr. Lisa Roberts has been with Western Colorado Physicians Group (WCPG), a division of Primary Care Partners, since 2015. She grew up on a farm in Iowa, and she attended the University of Iowa where she double-majored and earned her Bachelor of Science in Exercise Physiology with honors and Bachelor of Arts in History. Following this, she completed medical school at the University of Iowa and family practice residency and St. Mary’s Medical Center. During medical school and residency, she was fortunate enough to work in Bangladesh, the Indian Himalayas, and Guatemala. After residency, Dr. Roberts and her husband chose Grand Junction as their home.

Dr. Roberts cares for patients and families of all ages, from newborns to seniors. She strives to treat the whole person and focuses on prevention whenever possible.

Dr. Roberts loves the Grand Valley and all its wonderful outdoor opportunities. When she’s not at work, she can usually be found biking and camping.

Maxwell Stephens,
Doctor of Osteopathy

Dr. Maxwell Stephens joined Western Colorado Physicians Group in 2024. He was born and raised in St. Louis, Missouri. He completed his bachelor of science in biological sciences at the University of Missouri, and went on to study at the Kirksville College of Osteopathic Medicine for his medical training. Following medical school, he completed his residency and board certification in Family Medicine at St. Mary’s Medical Center in Grand Junction.

Medicine skipped a couple of generations in Dr. Stephens’ family. His great-grandfather was a practicing DO in rural Missouri, and Dr. Stephens grew up hearing stories about how his great-grandfather traveled to farms on horseback to deliver care (even babies), and he would deliver emergency care at his street corner office in town. When Dr. Stephens attended medical school, he would meet people who recognized him who would point out that his great-grandfather delivered their sister or took care of their mom. Seeing the impact his great-grandfather had on his community is what initially brought Dr. Stephens to study medicine.

Dr. Stephens’ medical interests are broad – including outpatient primary medicine, reproductive health, women’s health and obstetrics, LGBTQ+ health, preventive medicine, osteopathic manipulative medicine, rural and underserved healthcare, health policy development and many things in between. Outside of work, Dr. Stephens loves spending time outdoors camping, hiking, biking, paddleboarding, playing volleyball, skiing, cooking, and spending time with his family and dogs.

Seth Turner,
Doctor of Medicine

Dr. Seth Turner joined Western Colorado Physicians Group (WCPG), a division of Primary Care Partners, in 2021. He previously practiced family medicine in Tucson, Arizona, his hometown. Dr. Turner completed his family practice residency at St. Mary’s Medical Center, where he received the “L. Edward Ellingwood Award” for his commitment to underserved patients.

A committed and compassionate physician, Dr. Turner prides himself in partnering with his patients through strong communication and health promotion. His clinical interests include preventative care, public health, chronic disease management, advanced care planning, and geriatrics as well as pediatrics and women’s reproductive health. Dr. Turner thoroughly enjoys getting to know members of the community and is gladly accepting new patients.

Dr. Turner speaks Spanish and English. He spends his time away from work mountain biking and camping with his wife and daughter.

Victoria Cummings,
Doctor of Medicine

Known for her attentive listening and evidence-based care, Dr. Cummings is excited to rejoin Primary Care Partners at Western Colorado Physicians Group.

Dr. Cummings completed her undergraduate studies at Clarkson University in Northern New York in 2005, where she focused on Cognitive Neuroscience, Psychology, and Biology. After relocating to Colorado, Dr. Cummings pursued her medical degree through the University of Colorado Health Sciences Center. with a focus on rural family medicine she graduated in 2011 and completed her residency at St Marys, in Grand Junction in 2014.

Her clinical interests are deeply shaped by family life: her eldest son is on the autism spectrum, fueling Dr. Cummings’s commitment to neurodiverse-affirming care and guiding parents through complex diagnostic and treatment journeys.
Dr. Cummings is also passionate about women’s health throughout their lifespan. She deeply values her relationships with her obstetric patients and the rewarding journey of welcoming a new life into their families.

Outside the clinic, she recharges on Colorado’s trails with her two boys (ages 15 and 11) and their ever-excited corgi, Waffles. Whether hiking, practicing yoga, or mountain biking, she embraces the outdoors as a source of balance and resilience—qualities she brings into every patient encounter.

Kristi Haan,
Certified Physician Assistant

Kristi is a board-certified Physician Assistant (PA) practicing since 2000 at Western Colorado Physicians Group (WCPG), a division of Primary Care Partners. She received her Bachelor of Medical Science in Physician Assistant Studies from Midwestern University in Illinois and her Bachelor of Science in Biology from the University of Wisconsin-River Falls.

Kristi has always had a strong desire to pursue a career in medicine. Her grandfather was a family physician and inspired her passion to treat her patients as if they were members of her own family. Today, she manages patients with a variety of chronic and acute health conditions and strives to assist in providing quality, patient-focused healthcare.

Kristi enjoys hiking and paddleboarding when she’s not at work.

Jamie Kopfman,
Certified Physician Assistant

Jamie Kopfman is a Grand Junction native who joined Western Colorado Physicians Group (WCPG), a division of Primary Care Partners, in 2002. She received her undergraduate degree in Biology at Mesa State College and continued on to earn her Master’s in Physician Assistant Studies at Midwestern University in Glendale, Arizona. Jamie is also an adjunct professor for the Colorado Christian University Nursing Program.
Jamie earned her clinical specialty in family medicine and has a particular interest in women’s health and cardiology. She strives to see the unique physical, emotional, and mental aspects that contribute to the health of each individual and sees patients of all ages and walks of life.

When Jamie is not working, she spends time playing with her dog and hiking.

Tephi Mannlein joined Western Colorado Physicians Group (WCPG), a division of Primary Care Partners, in 2006. She graduated from Grand Canyon University in Phoenix, Arizona with a degree in Human Biology and earned her Master of Science in Physician Assistant Studies from Kirksville College of Osteopathic Medicine in Phoenix.

Tephi has clinical experience in family medicine, women’s health, and menopausal treatment. She is a member of the Wilderness Medicine Society, as well as a Certified Wound Care Specialist. Tephi has also been a caregiver at Docs on Call and Western Colorado Pediatrics Associates, both divisions of Primary Care Partners. She is committed to an outstanding patient experience and will go to great lengths to work toward her patient’s optimal health.

Outside of work, Tephi enjoys running, skiing, biking, camping, boating, cooking, and gardening.
