PATIENT PORTAL FAQs

Frequently Asked Questions about our Patient Portal, FollowMyHealth.

FollowMyHealth is a patient portal, available through the Primary Care Partners website, to provide a convenient and secure way for patients to manage their personal health records from any computer or mobile device with internet access.

Primary Care Partners does not store usernames and passwords. Please see FollowMyHealth’s step-by-step guide to reset your password. If you are still unsure of your login method, please contact your provider’s office.

The first priority is patient privacy and the safety of patient information. For that reason, we request that you provide us with an email address and Photo ID, before going to our website and signing up. We store and transmit all records via HIPAA compliant secure servers.

If you are using a mobile device, you must use the FollowMyHealth mobile app to get into your account by downloading the free app for your mobile device. Once you download the app, you can log in to your account using the same method you would if you were using the computer.

Connect to our facility. In your FollowMyHealth account click on “My Account” in the upper right-hand corner and then click on “Connections.” If you don’t see Primary Care Partners listed, you will need to click on “Add Connection.” Search by zip code 81501 for Primary Care Partners. Click on Primary Care Partners and click connect. The next step will be to accept the release of information. Once this is complete your connection is verified and once accepted you will see an email in 5 to 7 days to finalize the connection of your account.

In compliance with HIPAA laws and legal privacy, proxy access is available in three categories:

Minor Proxy access – Parents/guardians can set up a minor account allowing access to view and manage a child’s account until the child turns 12.
Young Adult Proxy access – Parents/guardians can have a limited Follow My Health account which allows secure messaging with the providers care team.
Adult Caregiver Proxy access – Medical Power of Attorney designees can request an account allowing access to view and manage a dependent adult accounts.

You will then receive an email invite. Click here for instructions on setting up an account with the email invite.
