Our care coordinators help patients to navigate the healthcare system and help to coordinate the team of healthcare professionals and community resources available to serve our patients. They assist our high-risk patients to effectively manage their health care needs. The purpose is to promote the best possible outcomes through coordinated efforts to manage their health conditions. Care Coordination includes coordinating patient care events intentionally and exchanging information with all participants associated with the care of a patient to achieve safer and more efficient care.
These are generally patients who receive care from multiple sources, either long-term or temporarily, or who use medical services more frequently.
We communicate with and connect the following:
- Medical Office Support
- Medical Assistant/Nurse Practitioners
- Home Health Agencies
- Community Resources
- Nursing Homes
Our care coordinators also help educate our patients about available Community Resources.
